Case Manager - intake and assessment (remote)

We are growing our team at Lumen Sable Early Learning and adding a Case Manager whose main responsibility will be intake and assessment. We celebrate small wins and steady routines. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for tuition and subsidies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with tuition and subsidies. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • Remote-friendly, with a monthly team day in Philadelphia. • Applications are reviewed on a rolling basis.
