Case Manager - intake and assessment (remote)

Stonegate Sable Community Partners is looking for a Case Manager. The job centres on intake and assessment for volunteer shifts, and it would suit someone who likes steady, practical work. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for volunteer shifts. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with volunteer shifts.
