Case Manager - intake and assessment (remote)

Fernwood Field Community Partners Group is looking for a Case Manager. The job centres on intake and assessment for donor records, and it would suit someone who likes steady, practical work. We are small enough to know everyone's name. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for donor records. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • Remote-friendly, with a monthly team day in Indianapolis. • We reply to every applicant.
