Case Manager - intake and assessment (remote)

Silverline Nest Community Partners is a nonprofit in San Jose supporting families with food, housing and job help, and we need a Case Manager to lead our intake and assessment. We are small enough to know everyone's name. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for food distribution. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with food distribution. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with a monthly team day in San Jose. • We reply to every applicant.
