Case Manager - intake and assessment (remote)

Greenridge Pine Community Partners Group is a Chicago charity working with residents on basic needs and opportunity, and we need a Case Manager to lead our intake and assessment. We are small enough to know everyone's name. What you will do • Help with applications and appointments for food distribution. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with food distribution. Working arrangement • Part-time, about 28 hours a week, afternoons. • Remote-friendly, with a monthly team day in Chicago. • Applications are reviewed on a rolling basis.
