Case Manager - intake and assessment (remote)

Westbrook Glen Care Services supports families and individuals in Dallas with personal care and coordination, and we need a Case Manager to lead our intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for caregiver training. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • Fully remote, with an occasional day on site in Dallas. • We will tell you what to expect at each step of the hiring process.
