Case Manager - intake and assessment (remote)

We are growing our team at Nova Ember Community Partners and adding a Case Manager whose main responsibility will be intake and assessment. Flexibility and kindness are expected. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Full-time, on a regular day shift. • Fully remote, with an occasional day on site in Denver. • We will tell you what to expect at each step of the hiring process.
