Case Manager - intake and assessment (remote)

We are growing our team at Lumen Vista Housing Group and adding a Case Manager whose main responsibility will be intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for waiting lists. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with waiting lists. Working arrangement • Part-time, about 20 hours a week, mornings. • Remote-friendly, with a monthly team day in Columbus. • Applications are reviewed on a rolling basis.
