Case Manager - intake and assessment (remote)

Bluepeak Ashton Health is looking for a Case Manager. The job centres on intake and assessment for clinic supplies, and it would suit someone who likes steady, practical work. Clinicians and administrators work side by side. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for clinic supplies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with the possibility of becoming permanent. • Remote-friendly, with an occasional day on site in Cincinnati. • We will tell you what to expect at each step of the hiring process.
