Case Manager - intake and assessment (remote)

Lumen Junction Community Partners is a Phoenix charity working with residents on basic needs and opportunity. We are hiring a Case Manager to take on intake and assessment. You would join a small organisation of about 21 people, working mostly from home. What you will do • Help with applications and appointments for grant reporting. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Part-time, about 20 hours a week, with flexible days. • Remote-friendly, with an occasional day on site in Phoenix. • Applications are reviewed on a rolling basis.
