Case Manager - intake and assessment (remote)

Northwind West Care Services supports families and individuals in Anchorage with personal care and coordination. We are hiring a Case Manager to take on intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for six months, with clear deliverables from the first week. • Fully remote, with a monthly team day in Anchorage. • We will tell you what to expect at each step of the hiring process.
