Case Manager - intake and assessment (remote)

Lumen Sable Early Learning operates neighbourhood childcare around Hartford, and we need a Case Manager to lead our intake and assessment. Children come first and so does looking after the teachers. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for licensing records. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with licensing records. Working arrangement • Part-time, about 24 hours a week, afternoons. • Remote-friendly, with an occasional day on site in Hartford. • We reply to every applicant.
