Case Manager - intake and assessment (remote)

Clearpath Union Health Group provides primary and specialty care in Detroit, and we need a Case Manager to lead our intake and assessment. Clinicians and administrators work side by side. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for electronic records. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Part-time, about 28 hours a week, with flexible days. • Fully remote, with a monthly team day in Detroit. • We will tell you what to expect at each step of the hiring process.
