Case Manager - intake and assessment (remote)

Rivermark Hollow Care Services is looking for a Case Manager. The job centres on intake and assessment for caregiver training, and it would suit someone who likes steady, practical work. We train and promote from within. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for caregiver training. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • Fully remote, with quarterly visits in Hartford. • We reply to every applicant.
