Case Manager - intake and assessment (remote)

Summit Loom Learning Group runs classrooms and tutoring across the San Diego area, and we need a Case Manager to lead our intake and assessment. We follow the school calendar and we like people who like learning. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for attendance records. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with attendance records. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • Remote-friendly, with an occasional day on site in San Diego. • Applications are reviewed on a rolling basis.
