Case Manager - intake and assessment (remote)

Stonegate Vista Community Partners is a nonprofit in Pittsburgh supporting families with food, housing and job help, and we need a Case Manager to lead our intake and assessment. You would join a small organisation of about 23 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for benefits enrolment. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with benefits enrolment. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Fully remote, with quarterly visits in Pittsburgh. • We will tell you what to expect at each step of the hiring process.
