Case Manager - intake and assessment (remote)

Cedar Forge Health Group is a healthcare provider in Albuquerque with clinics and support teams, and we need a Case Manager to lead our intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient scheduling. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • Remote-friendly, with quarterly visits in Albuquerque. • Applications are reviewed on a rolling basis.
