Case Manager - intake and assessment (remote)

Highbridge Rook Community Partners is a nonprofit in Pittsburgh supporting families with food, housing and job help. We are hiring a Case Manager to take on intake and assessment. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for grant reporting. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Fully remote, with an occasional day on site in Pittsburgh. • We reply to every applicant.
