Case Manager - intake and assessment (remote)

Rivermark Linden Health Group provides primary and specialty care in Charleston, and we need a Case Manager to lead our intake and assessment. We are busy and kind, and we look after each other. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient scheduling. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • Remote-friendly, with a monthly team day in Charleston. • We reply to every applicant.
