Case Manager - intake and assessment (remote)

Highbridge Health Group provides primary and specialty care in Des Moines. We are hiring a Case Manager to take on intake and assessment. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient scheduling. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Remote-friendly, with an occasional day on site in Des Moines. • We reply to every applicant.
