Case Manager - intake and assessment (remote)

Bright Union Health Partners provides primary and specialty care in Chicago. We are hiring a Case Manager to take on intake and assessment. You would join a small organisation of about 28 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for electronic records. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with a monthly team day in Chicago. • We reply to every applicant.
