Case Manager - intake and assessment (Raleigh, NC)

Lumen Rook Housing manages affordable housing communities across Raleigh, and we need a Case Manager to lead our intake and assessment. You would join a large organisation of about 615 people based in Raleigh. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for maintenance stores. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with maintenance stores. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Raleigh. • We reply to every applicant.
