Case Manager - intake and assessment (Raleigh, NC)

Rivermark Drift Health Group runs clinics and patient services around Raleigh, and we need a Case Manager to lead our intake and assessment. Clinicians and administrators work side by side. What you will do • Connect clients with partner agencies. • Help with applications and appointments for clinic supplies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies. Working arrangement • Full-time, Monday to Friday. • On site in Raleigh. • We will tell you what to expect at each step of the hiring process.
