Case Manager - intake and assessment (Raleigh, NC)

We are growing our team at Northwind Mill Health Group and adding a Case Manager whose main responsibility will be intake and assessment. You would join a small organisation of about 105 people based in Raleigh. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for clinic supplies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Raleigh, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
