Case Manager - intake and assessment (Raleigh, NC)

Stonegate Ivory Care Services is looking for a Case Manager. The job centres on intake and assessment for family communication, and it would suit someone who likes steady, practical work. You would join a small organisation of about 75 people based in Raleigh. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for family communication. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with family communication. • Bilingual English and Spanish.
