Case Manager - intake and assessment (Raleigh, NC)

Bluepeak Forge Health is looking for a Case Manager. The job centres on intake and assessment for patient education, and it would suit someone who likes steady, practical work. You would join a large organisation of about 450 people based in Raleigh. What you will do • Help with applications and appointments for patient education. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, mornings. • On site in Raleigh. • Applications are reviewed on a rolling basis.
