Case Manager - intake and assessment (Providence, RI)

Clearpath Junction Community Partners Group is a nonprofit in Providence supporting families with food, housing and job help. We are hiring a Case Manager to take on intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client intake. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Providence. • Applications are reviewed on a rolling basis.
