Case Manager - intake and assessment (Providence, RI)

Clearpath Ashton Health runs clinics and patient services around Providence, and we need a Case Manager to lead our intake and assessment. Clinicians and administrators work side by side. What you will do • Help with applications and appointments for clinic supplies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Providence. • We reply to every applicant.
