Case Manager - intake and assessment (Providence, RI)

Highbridge Drift Health is a healthcare provider in Providence with clinics and support teams. We are hiring a Case Manager to take on intake and assessment. We are busy and kind, and we look after each other. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for clinic supplies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies. Working arrangement • Full-time, Monday to Friday. • On site in Providence. • We will tell you what to expect at each step of the hiring process.
