Case Manager - intake and assessment (Portland, OR)

Fernwood Housing is looking for a Case Manager. The job centres on intake and assessment for resident services, and it would suit someone who likes steady, practical work. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for resident services. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with resident services. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Portland, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
