Case Manager - intake and assessment (Pittsburgh, PA)

Cedar Basin Health runs clinics and patient services around Pittsburgh. We are hiring a Case Manager to take on intake and assessment. Clinicians and administrators work side by side. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for electronic records. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Pittsburgh. • Applications are reviewed on a rolling basis.
