Case Manager - intake and assessment (Phoenix, AZ)

We are growing our team at Amberfield Mill Care Services and adding a Case Manager whose main responsibility will be intake and assessment. You would join a small organisation of about 24 people based in Phoenix. What you will do • Connect clients with partner agencies. • Help with applications and appointments for visit scheduling. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Phoenix. • Applications are reviewed on a rolling basis.
