Case Manager - intake and assessment (Phoenix, AZ)

Ironwood Keystone Care Services is a social-care provider serving older adults and people with disabilities around Phoenix. We are hiring a Case Manager to take on intake and assessment. The work is personal and so is the culture. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for caregiver training. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Phoenix. • We will tell you what to expect at each step of the hiring process.
