Case Manager - intake and assessment (Phoenix, AZ)

Fernwood Forge Civic Services is looking for a Case Manager. The job centres on intake and assessment for procurement, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for procurement. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with procurement. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Phoenix. • Applications are reviewed on a rolling basis.
