Case Manager - intake and assessment (Philadelphia, PA)

Ironwood Junction Housing Group is a nonprofit housing provider in Philadelphia. We are hiring a Case Manager to take on intake and assessment. You would join a large organisation of about 348 people based in Philadelphia. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for lease compliance. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with lease compliance. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Philadelphia, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
