Case Manager - intake and assessment (Philadelphia, PA)

We are growing our team at Greenridge Bridge Care Services and adding a Case Manager whose main responsibility will be intake and assessment. You would join a large organisation of about 924 people based in Philadelphia. What you will do • Connect clients with partner agencies. • Help with applications and appointments for caregiver training. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Philadelphia, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
