Case Manager - intake and assessment (Omaha, NE)

Redstone Quill Health Group runs clinics and patient services around Omaha, and we need a Case Manager to lead our intake and assessment. We are busy and kind, and we look after each other. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for patient education. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Full-time, Monday to Friday. • On site in Omaha, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
