Case Manager - intake and assessment (Omaha, NE)

Ironwood Linden Care Services supports families and individuals in Omaha with personal care and coordination. We are hiring a Case Manager to take on intake and assessment. We train and promote from within. What you will do • Connect clients with partner agencies. • Help with applications and appointments for client records. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • On site in Omaha. • We will tell you what to expect at each step of the hiring process.
