Case Manager - intake and assessment (Oakland, CA)

Stonegate Mill Care Services Group is looking for a Case Manager. The job centres on intake and assessment for licensing, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in Oakland. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for licensing. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Oakland, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
