Case Manager - intake and assessment (Oakland, CA)

Rivermark Hollow Care Services is a social-care provider serving older adults and people with disabilities around Oakland. We are hiring a Case Manager to take on intake and assessment. We train and promote from within. What you will do • Help with applications and appointments for family communication. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, afternoons. • On site in Oakland. • We will tell you what to expect at each step of the hiring process.
