Case Manager - intake and assessment (Oakland, CA)

We are growing our team at Clearpath Drift Health Group and adding a Case Manager whose main responsibility will be intake and assessment. You would join a large organisation of about 212 people based in Oakland. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient scheduling. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Full-time, Monday to Friday. • On site in Oakland. • Applications are reviewed on a rolling basis.
