Case Manager - intake and assessment (Nashville, TN)

Foxglove Umber Health is a healthcare provider in Nashville with clinics and support teams. We are hiring a Case Manager to take on intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for clinic supplies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Nashville. • We will tell you what to expect at each step of the hiring process.
