Case Manager - intake and assessment (Nashville, TN)

We are growing our team at Highbridge Basin Health Group and adding a Case Manager whose main responsibility will be intake and assessment. Clinicians and administrators work side by side. What you will do • Help with applications and appointments for electronic records. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Full-time, on a regular day shift. • On site in Nashville. • We will tell you what to expect at each step of the hiring process.
