Case Manager - intake and assessment (Nashville, TN)

We are growing our team at Fernwood Vista Community Partners and adding a Case Manager whose main responsibility will be intake and assessment. We are small enough to know everyone's name. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for client intake. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Nashville. • We will tell you what to expect at each step of the hiring process.
