Case Manager - intake and assessment (Nashville, TN)

Lumen Fox Learning Group is looking for a Case Manager. The job centres on intake and assessment for family communication, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Nashville, with some flexibility after the first few months. • We reply to every applicant.
