Case Manager - intake and assessment (Nashville, TN)

Redstone Birch Care Services is a social-care provider serving older adults and people with disabilities around Nashville. We are hiring a Case Manager to take on intake and assessment. Teams support each other, especially on hard days. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for care plans. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Full-time, with a steady weekday schedule. • On site in Nashville. • We reply to every applicant.
