Case Manager - intake and assessment (Minneapolis, MN)

Highbridge Wren Care Services is a social-care provider serving older adults and people with disabilities around Minneapolis. We are hiring a Case Manager to take on intake and assessment. You would join a large organisation of more than 1,000 people based in Minneapolis. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for caregiver training. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with caregiver training.
