Case Manager - intake and assessment (Minneapolis, MN)

Bluepeak Ember Community Partners runs community programmes across Minneapolis. We are hiring a Case Manager to take on intake and assessment. Flexibility and kindness are expected. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for donor records. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Minneapolis. • We will tell you what to expect at each step of the hiring process.
