Case Manager - intake and assessment (Minneapolis, MN)

Fernwood Lantern Community Partners runs community programmes across Minneapolis. We are hiring a Case Manager to take on intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for client intake. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Minneapolis. • We will tell you what to expect at each step of the hiring process.
