Case Manager - intake and assessment (Milwaukee, WI)

Nova Rowan Health is a healthcare provider in Milwaukee with clinics and support teams, and we need a Case Manager to lead our intake and assessment. We are busy and kind, and we look after each other. What you will do • Help with applications and appointments for clinic supplies. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in Milwaukee. • Applications are reviewed on a rolling basis.
